Population Health Management (PHM)

Population Health Management (PHM) provides members with a comprehensive population health approach that prioritizes prevention, early intervention, whole person care, brings networks together, partners within the community to address social determinants of health (SDOH), and provide access and quality of health care across the continuum.

PHM is focused on building the strengths and resiliencies of our members, while addressing their needs and preferences. This is provided through case management and care coordination. Members who qualify for PHM services may receive one or more of the following services in the following categories:

  • Care Management
    • Basic Population Health Management
    • Complex Case Management Services
  • Transition of Care Services
  • Palliative Care Services
  • Major Organ Transplant
  • Long-Term Care
    • Skilled Nursing Facility (SNFist) Provider Program
  • Baby Steps and Baby Steps Plus
  • Kids and Youth (KAY)and Transitional Program
  • Chronic Obstructive Pulmonary Disease (COPD) Program
  • End Stage Renal Disease (ESRD) Program
  • Long Term Support Services and Support (LTSS)
  • Member Centric Care Coordination

The services offered by PHM is at no cost to KFHC Members. Our PHM team members may call you if are referred to us and qualify for the program. You may also call us at 661.664.5000 to determine eligibility for each PHM Programs. You can also talk to your doctor or clinic staff to see if you qualify.
