Population Health Management (PHM) provides members with a comprehensive population health approach that prioritizes prevention, early intervention, whole person care, brings networks together, partners within the community to address social determinants of health (SDOH), and provide access and quality of health care across the continuum.
PHM is focused on building the strengths and resiliencies of our members, while addressing their needs and preferences. This is provided through case management and care coordination. Members who qualify for PHM services may receive one or more of the following services in the following categories:

- Transition of Care Services
- Palliative Care Services
- Major Organ Transplant
- Long-Term Care
- Skilled Nursing Facility (SNFist) Provider Program
- Baby Steps and Baby Steps Plus

- Chronic Obstructive Pulmonary Disease (COPD) Program
- End Stage Renal Disease (ESRD) Program
- Long Term Support Services and Support (LTSS)
- Member Centric Care Coordination
