HHP benefits include:

What do I need to join?

1. Have Medi-Cal coverage and be enrolled in a Medi-Cal plan.
2. Have specific chronic health problems (like asthma, diabetes, high blood pressure, kidney or liver failure, heart failure, etc.).
3. Have been in the hospital or emergency room or be chronically homeless.

Joining the HHP will not change any Medi-Cal benefits.
