As a Kern Family Health Care (KFHC) member, you can get rewards for your health actions. See how many rewards you may qualify for in the following list.

New members: $25

Complete a wellness visit within 120 days of enrolling with KFHC.

Annual well care: $25

Complete a yearly wellness exam between 3 and 21 years of age.**

Baby well care: Up to $160

Complete six well-baby visits between 0 and 15 months. Earn $15 each for the first four visits. Earn $25 each for the last two. Complete two well-baby visits between 15 and 30 months. Earn $25 for each visit.

Blood lead screening: $25

Complete a blood lead test before your baby turns 2 years of age.

Prenatal: $50

Visit your doctor within the first three months of becoming pregnant and submit the Prenatal Reward Form.*

Postpartum: $30

Visit your doctor for a postpartum visit within 1 and 12 weeks after giving birth.

Breast cancer screening: $25

Women 50 to 74 years of age, complete a mammogram.**

Cervical cancer screening: $25

Women 21 to 64 years of age, complete a Pap smear test.**

Chlamydia screening: $25

Women 18 to 24 years of age, complete a test for chlamydia.**

Follow-up for mental health emergency room visit: $25

Members 6 years of age and older, visit your doctor within 30 days after being seen for mental illness in the emergency room.**

Follow-up for substance abuse emergency room visit: $25

Members 13 years of age and older, visit your doctor within 30 days after being seen for substance abuse in the emergency room.**

Hemoglobin A1c blood test screening: $25

Members 18 to 75 years of age with diabetes, complete a blood test screening.**
