By making healthy choices, you can help minimize your risk for certain diseases and conditions. That's why we offer you certain preventive care programs and guidelines that can help keep you on the right track.

Our Preventive Care Guide explains how to maintain your and your children's health. We ask that all Kern Family Health Care members read the guide to learn about the recommended health services for each stage of life. The guide is available in both English and Spanish.

Join the Diabetes Prevention Program

For members at risk for diabetes, we offer our Diabetes Prevention Program (DPP) that teaches lifestyle changes designed to prevent or delay type 2 diabetes. During our one-year DPP, you'll:

  • Work with a DPP coach.
  • Learn self-monitoring and problem solving.
  • Receive encouragement and feedback.
  • Learn to support your goals.
  • Track routine weigh-ins.

To qualify for DPP, members must have been diagnosed with prediabetes by their provider and meet the rest of the program's eligibility requirements. To learn more about your eligibility and how to enroll, call 800.391.2000 and ask for the Disease Management Department.
