Member rewards programs

We know that taking care of your health and the health of your family takes effort, and we believe that effort should be rewarded. That's why we offer a variety of member rewards programs that reward you for making healthy choices. New rewards amount effective November 1, 2022.

For new members

Complete your Initial Health Assessment (IHA) within 120 days of enrolling to receive a $25 gift card. The IHA is a complete health visit with your doctor. It helps your doctor get to know you and manage your health.

Once you complete this visit, we will wait for your doctor to bill us for the visit to see if you qualify. We will mail your gift card to the mailing address on file for you at KFHC. Please log in to your Member Portal account or call KFHC to make sure we have the correct address on file. Please note that it may take up to 6-8 weeks for us to process your reward.

For pregnant members

Pregnant members may receive rewards* for early prenatal and timely postpartum care. There are the two ways to receive these rewards.

1. Early prenatal care. Visit your doctor for a prenatal care visit during your first trimester to receive a $50 gift card in the mail. To receive this reward, have your doctor complete and sign the Prenatal Reward Form (English and Spanish) and return it to us by mail or fax.
2. Timely postpartum care. Visit your doctor within 1 to 12 weeks after giving birth to receive a $30 gift card in the mail. You don't need to submit any forms to receive this reward. Once you complete this visit, we will wait for your doctor to bill us to see if the visit qualified.

*Pregnancy rewards are limited to two gift cards total per pregnancy.

For preventive care and wellness visits

Members who seek out regular care may qualify for these rewards.

• 0-30 months old. Babies need wellness visits to help keep them healthy. Complete 6 well baby visits between 0-15 months of age and 2 more well baby visits between 15-30 months. Receive a $15 gift card for 1st, 2nd, 3rd, 4th, visit and $25 for 5th, 6th, 7th, and 8th visit. That adds up to $160 in gift cards!
• 3-21 years old**. Yearly wellness exams help keep you healthy and strong. These visits can help prevent and manage chronic health conditions. For each visit you complete between the ages of 3-21 years old, you will receive a $25 gift card.
• Blood lead screening**. Lead is a harmful toxin, especially among young children. A blood lead test can help detect the toxin and start care if needed. Complete a blood lead test before your baby turns 2 years of age. Receive a $25 gift card once your baby gets tested.
• Breast cancer screening**. Women 50-74 years of age should get screened for breast cancer. Completing a breast cancer screening can help detect cancer early when it’s easier to treat. Complete a mammogram and receive a $25 gift card.
• Cervical cancer screening**. Women 21-64 years of age should get a Pap Smear every three years. This test makes it easier to find cancer at an early stage and treat it. Complete a Pap Smear and receive a $25 gift card.
• Chlamydia screening**. Women 18-24 years of age should get screened for Chlamydia once a year. Complete this simple test and get a $25 gift card.
