We want every plan member to have the tools they need to live a healthy life. That starts with your initial health assessment (IHA) and continues with our programs and education services. Discover how we support you on your health journey.
Complete your initial health assessment
All new members are encouraged to see their Primary Care Provider (PCP) for an initial health assessment. This first visit with your PCP is important. It's the time to get to know each other and review your health status or your child's health status. Your PCP will help you understand your or your child's medical needs and advise you on healthy options.
It is very important that you see your PCP and that your child sees their PCP for an IHA within 120 days of becoming a Kern Family Health Care member.
Plus, when you complete your IHA on time, we'll send you a free gift as a thank you for staying on top of your healthcare. Call your PCP's office for an appointment today.
