Kern Family Health Care (KFHC) provides Community Supports Services (CSS) for members that meet the needs of the CSS program. CSS is extra care that can help you with social needs such as housing and food. CSS can help you stay healthy and stay out of the hospital and emergency room.
Members who meet the needs of the CSS program can get help with finding housing. CSS can help with housing needs, to get food, and to have a safe and clean place to stay after a hospital visit.
You may be called from the CSS program if you meet the needs to join. You can always call us sooner to see if you can join at 661.632.1590 option 6 or 1.800.391.2000 option 6, or speak to your doctor or clinic staff to see if you meet the needs to join the CSS program.
Covered Community Supports Services (CSS)
KFHC works with local partners and community service offices to help you get the support you need for housing, food, and other medical support.
This program will assist you with finding housing by helping you fill out the rental or lease agreement and the needed paperwork. Plus, we can help with the deposit to secure your rental.
This is a 1-time housing payment program that may help pay for your rent deposit. We can help with the fees to turn on the gas and power. Our team can help set up your medical equipment and cover basic household expenses.
Recuperative care, also called medical respite
If you need help to get well after your hospital stay, this program will help by sending you to a short-term care home or a center where you can get the care you need to get back on your feet.
If you have asthma, this program may provide healthy changes to your home or where you live, such as air purifiers or other medical supplies.
Short-term housing after a hospital stay
This program will help you find housing after your hospital stay.
KFHC works with local partners and community service offices to help arrange these services for you. If you meet the program's needs, here is a list of who we have partnered with:

Cost to member
There is no cost to the member for CSS services.

You may be eligible to join the CSS program if you are homeless or might become homeless, have complex health, disability, and/or behavioral health conditions.
