Description: Under the direction of the Manager of Health Education and Disease Management and Director of Quality Improvement, Health Education and Disease Management coordinates and implements Kern Health Systems’ (KHS) Disease Management Program. The Disease Management Case Manager is a full time employee who is responsible for the coordination, collaboration, education and management of members identified to be in the high risk category of Asthma and Diabetes. The Case Manager functions within the Health Services Department of a Knox-Keene licensed health maintenance organization (HMO).

Requirements: Possession of a valid California Registered Nursing License required. Bachelor’s degree from an accredited school preferred. Five (5) years of experience as a registered nurse, at least three (3) years in an acute care hospital on a medical/surgical ward or unit or in the medical/surgical intensive care unit preferred. Patient education and previous utilization management experience in a managed care setting preferred. Equivalent experience or education may be substituted for above requirement. Plan, coordinate, implements, and report all aspects of the Disease Management program; prepare concise reports; communicate effectively with members, interdepartmentally, and contract providers. Preferably, ability to collect and analyze data, including the use of database applications. Valid California Driver’s License and proof of valid, legally mandated, auto liability insurance.
