Request a Replacement Member ID Card
You may request a new ID card if you have lost yours by:

  1. Calling Member Services at 1-800-391-2000
  2. Submitting the attached request at the link below and filling out the requested information completely. After filling out the entire form click the SUBMIT button.

    ONLINE ID CARD REQUEST

After submitting your request you should receive your card within seven business days.
